Brittany Murphy — Change On Death Certificate

More than a year after Brittany Murphy’s death, officials have finally added her father’s name to her death certificate … after the guy complained he was being blocked from visiting his daughter’s grave because of doubts he was a relative.

When Brittany died back in 2009, the death certificate read “Unknown” in the box asking for the name of her father — this, despite the fact Angelo Bertolotti is listed as the father on Brittany’s BIRTH certificate.

Angelo was married to Brittany’s mom … but they divorced when Brittany was 2-years-old.

Angelo tells TMZ … initially he wasn’t upset with the snub … but he subsequently got angry when the staff at Forest Lawn cemetery wouldn’t allow him to visit Brittany’s grave site because he couldn’t prove he was family.

So Angelo petitioned for a change — and after bringing his I.D., photos of himself with Brittany and Brittany’s birth certificate to the L.A. County Registrar … his name was belatedly added to her death certificate.

Angelo tells us he’s recently visited his daughter’s grave several times … without incident.

Brittany Murphy’s Mom — Shared Bed with Monjack

Brittany Murphy’s mother told investigators that she shared a bed with her daughter’s husband Simon Monjack after Brittany passed away … this according to sources close to the investigation.

The L.A. Coroner’s Office just released their report on Simon Monjack’s death.

In the report, the investigator documents an exchange with a person whose name has been withheld — but whom law enforcement sources have identified as Brittany’s mother, Sharon Murphy.

According to the report, the investigator and the unnamed person (Sharon) were going through the master bedroom in the Hollywood Hills home that Simon shared with Brittany … when (Sharon) pointed to one side of Simon’s bed and identified it as “her side of the bed.”

Sharon also told the investigator that certain prescriptions in the nightstand — bearing the names Sharon Murphy and Sharon Monjack — were also hers.

The report doesn’t suggest sexual contact between the two.

Brittany Murphy (November 10, 1977 – December 20, 2009), was an American actress and singer. She starred in films such as Clueless; Just Married; Girl, Interrupted; Spun; 8 Mile; Uptown Girls; Sin City; Happy Feet; and Riding in Cars with Boys. She voiced Luanne Platter on the animated TV series King of the Hill. Her final film, Abandoned, is expected to be released direct-to-video in August 2010.

Brittany attended Verne Fowler School of Dance and Theatre Arts in Colonia, New Jersey, in 1982. At the age of 4, she continued to train in singing, dancing, and acting until her move to California at 13. Brittany made her Broadway debut in 1997, as Catherine, in a revival of Arthur Miller’s A View From The Bridge opposite veteran actors Anthony LaPaglia and Allison Janney.

Brittany was in a band called Blessed Soul with fellow actor Eric Balfour in the early 1990s. On June 6, 2006, Murphy and Paul Oakenfold released the single “Faster Kill Pussycat”, from the album A Lively Mind. The song became a club hit, and hit number one on Billboard’s Hot Dance Club Play chart. It also hit number seven in the UK singles chart in June 2006.

At 08:00 (16:00 GMT) on December 20, 2009, the Los Angeles Fire Department responded to “a medical request” at the Los Angeles home Murphy and Monjack shared. Brittany had apparently collapsed in a bathroom. Firefighters attempted to resuscitate Murphy on the scene. She was transported to Cedars-Sinai Medical Center, where she was pronounced dead on arrival at 10:04 after going into cardiac arrest.

Brittany Murphy, Husband’s Deaths — the Mold Factor

The investigation into Brittany Murphy and Simon Monjack’s deaths is now being handled by the Department of Public Health, TMZ has learned … and it’s all because of mold found in their home.

Law enforcement sources tell TMZ the issue of mold in the house came up early during the investigation into Brittany’s death — but it was deemed “not to be a contributing factor.”

Then when Simon passed away a few months later, we’re told the issue came up again — but Brittany’s mom, Sharon, wouldn’t let the L.A. County Coroner’s Office in to investigate the mold. Now, we’re told, Sharon has agreed to let Public Health investigators into the home.

Mold could be relevant to Simon and Brittany’s respiratory health. Pneumonia was a cause of death for both of them.

We’re also told Simon’s mother, Linda Monjack is fully supportive of this move, and will be flying from Europe so she can be close to the investigation.

Simon Monjack Dies From Same Cause as Brittany

TMZ has learned … Brittany Murphy’s husband, Simon Monjack, died from acute pneumonia and severe anemia.

The L.A. County Coroner found prescription medications in Simon’s blood stream but not enough to be lethal.

Ironically, Brittany died 5 months earlier of pneumonia, anemia and multiple drug intoxication.

We’ve learned Simon’s family in England was advised of his cause of death earlier this morning by the L.A. County Coroner’s Office.

Simon Monjack — Death Clue in Voicemail

TMZ has learned law enforcement officials are investigating a mysterious clue in the Simon Monjack death case — a curious voicemail that Simon left on his own cell phone.

Sources close to the case tell TMZ when investigators went through Simon’s cell phone, they found a message in his voicemail box that was created on the day he died … in which Simon can be heard saying, “I need to go to the hospital. I have a fever.”

Later on the recording, Brittany’s mother Sharon can be heard saying, “Tell them you have a … fever.” It’s unclear who “them” is.

We’re told investigators are unclear if Monjack pocket-dialed his own voicemail … intentionally called his cell phone from another line …. or was so disoriented, that he accidentally dialed the wrong number in an attempt to call for help.

Monjack died on May 23 — his cause of death has yet to be determined.

Simon Monjack’s Secret — Kids

TMZ has learned …a laptop computer has unearthed a secret Simon Monjack carried to his death — he had two kids.

Simon’s mom, brother and therapist stayed at Simon’s home last week with Brittany Murphy’s mom, Sharon Murphy.  We’re told when they left they took Simon’s laptop.

At some point they went through the computer and found emails from two women who wanted money from Simon for child support. According to people who have reviewed the emails, Simon had a son by one baby mama and a daughter by another.

Also in the computer … wire transfers of large amounts of money — as much as $48,000 in a single transfer.  Simon was sending the money to lawyers, as well as someone in Europe for some sort of settlement.

We’re told Simon’s mom and brother felt the coroner should have the computer and handed it over.  The coroner’s office is currently reviewing the contents.

Sharon went ballistic and filed a theft report with cops.

Simon Monjack — Cause of Death Still a Mystery

We just obtained the death certificate of Brittany Murphy’s husband, Simon Monjack … and the cause of death has not been determined.

According to the document, the cause of death is “deferred.” The certificate lists Monjack as “film maker” … in “entertainment.” It also notes his marital status — “widowed.”

The death certificate lists Monjack as 40 years old.

Simon Monjack delayed having heart surgery as he grieved for Brittany Murphy.

The scriptwriter – who died suddenly on Sunday (23.05.10) aged 39, just five months after his actress wife passed away last December aged just 32 – is believed to have been trying to put off a bypass operation for his condition until after a planned charity event in September for the Brittany Murphy Foundation.

His publicist Roger Neal said: “I can tell you he was excited about what the future held in terms of the legacy of Brittany Murphy. He looked fine to me.

“As far as we know he passed away of natural causes. He did have a heart condition and he was due for bypass heart surgery.”

The writer was found unconscious at the home he shared with the late ‘Clueless’ star by her mother Sharon Murphy – who also discovered Brittany’s body in the shower of the home.

Los Angeles County Assistant Chief Coroner Ed Winter has previously revealed prescription drugs were found near Monjack’s body, although he has refused to give any further details.

Sharon is “devastated” to lose her son-in-law.

Roger said: “It is a very sad day for Ms. Murphy. She’s devastated. She’s a very strong woman. I don’t know how you handle something like that. She is resting. Sharon loved Simon as a son. They were a very close family.”

At the time of his passing, Simon had been setting up a charity to pay for dance and singing lessons for youngsters who can’t afford private tuition.

Roger added: “He and Sharon we both very involved in a foundation being set up to help children. He was getting ready for a whole lot of things that were very positive and very incredible… Just talking with him and Sharon, the expressions on their face, they were still sad over the loss of Brittany, which is to be expected, but he was very focused about what they were doing with this formation of the foundation and the gala and things like that.

“Brittany’s dream, as was Simon’s, was to help children who want to learn to do ballet, sing, etcetera, who couldn’t afford it. The foundation would raise money for it.”

The official cause of Monjack’s death is still to be determined. An autopsy and toxicology testing should take place in the next few days.

Simon Monjack — Possible OD

Law enforcement sources tells TMZ there are signs Simon Monjack may have died from an accidental overdose of prescription drugs.

One well-placed source says, “There were a number of pill bottles on the nightstand in the bedroom, and some were empty.”

There is, obviously, an eerie similarity to the death of Monjack’s wife, Brittany Murphy, just 5 months ago.  In her case an array of prescription drugs were found on the nightstand of the same bedroom where Monjack died.  Many of the drugs on Simon’s nightstand were the same as the ones found after Brittany died.

One law enforcement source says the two causes of death that seem most plausible at the moment are “natural” and “accidental drug overdose.”

Monjack told TMZ recently he had suffered a heart attack during the incident last November when he had a medical emergency aboard an airplane.

Brittany’s mother, Sharon Murphy, discovered Monjack’s body last night.  She is also the one who found her daughter, Brittany, unconscious and on the floor of the bathroom 5 months earlier.

Brittany Murphy’s widower has been found dead.

Simon Monjack – who was left devastated when his actress wife died last December aged just 32 – was discovered unconscious by Brittany’s mother Sharon at his Los Angeles home last night (23.05.10) at around 9.30pm.

She immediately dialled 911 but Simon, 39, was pronounced dead by paramedics after attempts to resuscitate him were unsuccessful.

It is believed he suffered a heart attack and his passing is being treated by Los Angeles police as unexplained.

A death investigation is now underway, according to website TMZ.

Following Brittany’s death on December 20, the British screenwriter – who was left out of her will – had spoken out about their life together saying they were planning to start a family.

He said: “She knew her career wasn’t in the best place. She was ready to kind of be a mother and that was what a lot of the talk was about.

“We were discussing children the night before she passed. We were discussing baby names. A boy would have been Oliver and a girl would have been Sophie. That was the plan, to move to New York and to start a family.”

He also revealed how he felt “vindicated” by her autopsy report after the enquiry into her death showed she had no history of drug abuse.

He said: “Although Brittany’s autopsy report was incredibly shocking and sad to the family – her mother Sharon Murphy and I both shed tears – we are vindicated in that we always said Brittany never did any kind of illegal substances and that the medication she was taking was the medication found in her system. We hope now that people will understand the legacy of Brittany Murphy and the incredible body of work of Brittany Murphy.

“I am so sad that we are living in a culture of rumour and I just wish that people would understand that sometimes out of every tragedy can come some good”

Brittany Murphy’s husband and mother were questioned by coroners at their home yesterday (03.02.10).

The late ‘Clueless’ actress’ spouse Simon Monjack and her mum Sharon Murphy – who lived with the couple in Los Angeles – were visited by Assistant Chief Coroner Ed Winter and queried about the circumstances surrounding the 32-year-old star’s death on December 20.

Leaving the building, he told TMZ: “The investigation is continuing. The family is very cooperative and we will continue with our investigation.

“They were able to fill in some questions that we had involving the case. But I really can’t comment on it.

“We actually interviewed them together.”

Brittany is believed to have died from a suspected cardiac arrest, but her death certificate lists the cause of her death as “deferred” and the manner of death as “pending investigation”.

Following her passing, there has been much speculation about Brittany’s use of prescription drugs and weight issues.

Ten different types of medication were confiscated from her bedroom after she was found collapsed in the shower.

It is believed the coroner paid the pair a visit to find out more information after being unsatisfied the toxicology report – which has not yet been released – told the full story.

This news comes shortly after it was claimed the coroners had been warned prescription medications used to arrive at the property every week, addressed to someone called ‘Lola’.

The gossip website reports: “Sources tell TMZ a man who worked at Brittany Murphy’s house has told the Coroner’s Office that someone would deliver a sealed, manila envelope to the house every Friday filled with prescription medications in bottles. The name on the envelope – ‘Lola.’

“The ex-employee told the coroner drugs were also mailed from Los Angeles to Louisiana – where Brittany was shooting a movie and Simon was tagging along – in the months before she died.”

The official cause of death is due to be announced shortly, when toxicology test results come back and the autopsy report will be released.

The actress – who died at the age of 32 on December 20 following a cardiac arrest – provided the vocals for DJ Paul Oakenfold’s 2006 dance track ‘Faster Kill Pussycat’ but wasn’t keen to be credited on the song.

Paul revealed: “I said to her, ‘Why hide behind something? You have a really great vocal. People are going to be shocked and pleasantly happy with what they’re going to hear. You are not one of these actresses who are trying to sing and not doing a good job of it.’

“I demoed three other girls on that song – and some of the girls were pretty big singers – and Brittany blew them away.”

The Grammy-nominated artist also revealed Brittany was in the process of recording her own album and says it’s the world’s loss they won’t ever be able to hear it.

He told People magazine: “She would have done well. She wanted to try this with me, this smoky blues sound with electronic rhythms. She wanted to sound more traditional originally but I told her we should go more edgy. She was really excited by that and it’s just a shame it never got as far as people thought it could. It’s just a shame that the world is not going to hear more songs of Brittany Murphy.”

Brittany was laid to rest on Christmas Eve (24.12.09) at the Forest Lawn Memorial Park, in Hollywood.

Her husband Simon Monjack gave a eulogy in which he described her as his “soul mate” and the service was overseen by a Christian minister and a rabbi and included the hymn ‘Amazing Grace’.

After the private ceremony, her family released a statement which read: “Brittany was an incredibly loving and passionate person and an artist to her core – she loved acting, singing, dancing and performing.

“A bright light that lit the world is forever dimmed, but will live on in the hearts of those that Brittany touched.”

He said: “I was very shocked by her death. I can’t believe it really. It happened so fast, I’m just trying to figure it all out right now. I’m very upset by it.

“If I wanted to go to the funeral I would go, but I don’t want to see her that way. She was flawless to me. She was a little bright child. I have only good memories about her. She’s a memory to me now. To me she’s off making a movie somewhere.”

Angelo hadn’t spoken to his daughter for over a year and never met her British screenwriter husband Simon Monjack – who she wed in 2007 following a whirlwind romance.

Since she died, there has been much speculation about Brittany’s use of prescription drugs and weight issues.

A collection of medication and empty pill bottles – including tablets to treat depression and anxiety – have allegedly been confiscated by an officer from the Los Angeles County Coroner’s department who is investigating the 32-year-old actress’ death after she was found dead in the shower on Sunday (20.12.09).

The drugs were in Brittany’s name, her husband Simon Monjack’s, her mother Sharon’s and other “unidentified third party names”.

They included anxiety pills Klonopin and Atvian, Carbamazepine; which treats diabetic symptoms and is also used to help people who suffer from bipolar disorder, painkillers Vicoprofen and Hydrocodone and antibiotic tablets Biaxin.

There was also a packet of Propranolol; for hypertension and to prevent heart attacks, anti-seizure medication Topamax, anti-inflammatory treatment Methylprednisolone, depression medication Fluoxetine and several different vitamins.

Toxicology expert Dr. Bruce Goldberger – Professor and Director of Toxicology at the University Of Florida College Of Medicine – has confirmed a mixture of these medicines could have resulted in an accidental overdose.

He said: “Absolutely, mixing a number of these drugs could have resulted in her death. At the moment it is pointing towards an overdose or at least that will have to be ruled out by the autopsy or toxicology results.”

Brittany’s autopsy took place on Monday (21.12.09) but the finding will not be released until toxicology and neurological results come back in the next six weeks.

Meanwhile, Simon, 39, has blasted speculation his wife was also taking illegal drugs and suffering from an eating disorder.

He told the New York Daily News newspaper: “I’m confident it wasn’t an overdose. She was not a drug addict.

“My wife did not take anything on the morning of her death that would have caused her to be sick or ill, let alone die. It’s beyond even the sickest speculation.

“The attacks on me, the attacks on my wife and suggestions that she was anorexic and drug addicted, it’s like, look at the recent photographs. That’s not the smile of an anorexic or drug addict or someone in a bad marriage. That’s someone who was content with her life.

“She was a powerful, strong beautiful woman, despite being my baby and her mother’s life.”

Brittany reportedly left her estate to her mother in a will she wrote before marrying Simon in 2007. It is unclear if she had amended it.

British screenwriter Simon Monjack will be one of several people questioned as authorities try to determine how the actress died from cardiac arrest on Sunday (20.12.09), aged just 32.

Simon – who reportedly tried to stop the Los Angeles Coroner’s Office from performing an autopsy on Brittany’s body yesterday (21.12.09) – previously said he has been “destroyed” by her passing.

Meanwhile, friends of the couple – who wed in 2007 after a whirlwind romance – have spoken out about their concerns surrounding their relationship and their belief he is riddled with huge debts.

It has been reported a 2006 court judgment for £293,000 was made against him by the prestigious Coutts and Company bank, and it is claimed he was evicted from his New York flat in 2005 after failing to pay £5,000 in rent.

US website Gawker has reported there are two outstanding warrants for his arrest in Virginia for alleged fraud and credit card theft and other media outlets claim he was arrested by customs agents in April 2007 for overstaying his work visa.

Simon said: “Like all businessmen, I have faced many lawsuits. It is a real tragedy that success, greed, envy and celebrity are the motivation for attacking my family and character.”

Meanwhile, ‘Clueless’ director Amy Heckerline has added to speculation Brittany suffered from anorexia – after revealing she felt under pressure to be a “skinny, pretty girl”.

Amy told Scott Feinberg’s AndTheWinnerIs.blog.com: “You know, everybody’s shaking their heads and going ‘drugs,’ of course… She seemed to go through a change, you know, on ‘Clueless.’ Maybe she felt like she was not the, like, skinny, pretty girl, you know?

“And then the next few movies she was thinner, blonde, at MTV Awards and all sorts of things like that, and going out with Eminem and Ashton Kutcher.

“I think she felt the pressure to become a different sort of commodity to survive in show business, and I think it was awful.”

The autopsy on her body was completed yesterday (21.12.09) but results will not be announced until toxicology and neurological tests come back during the next six weeks.

The late ‘Clueless’ actress’ spouse, British screenwriter Simon Monjack, has spoken out about his beloved wife – who died of a cardiac arrest at her Los Angeles home aged just 32 on Sunday (20.12.09) – and his devastation at her passing.

He said: “My world was destroyed yesterday.”

Simon also revealed life had been normal for the pair until she was found collapsed in the shower surrounded by her own vomit, although Brittany had been suffering from a sore throat recently.

Recalling how her lifeless body was discovered, he explained to ‘Access Hollywood’: “Her mother Sharon went into the bathroom because she had been in there a long time. Her mom screamed for me and I ran. Then I called 911.

“She had laryngitis. She had been tired at the end of the year. She had made a couple of movies.”

Simon has also denied rumours Brittany had been mixing with “controlling” people.

He explained: “I don’t know why anyone would think that. She found love. We found love. Brittany didn’t get to where Brittany was with anyone controlling her… Brittany was Brittany.”

Simon also praised his wife’s ex-boyfriend Ashton Kutcher’s moving tribute to the actress.

He added: “I loved what Ashton Kutcher wrote on twitter. It was comforting to me. I couldn’t have said it better.”

Ashton had previously written: “2day the world lost a little piece of sunshine. My deepest condolences go out 2 Brittany’s family, her husband, and her amazing mother Sharon. see you on the other side kid.”

Following the actress’ death, it was reported Simon didn’t want Brittany to have an autopsy – which was completed yesterday (21.12.09).

The Los Angeles County Coroner’s Office is waiting for the results of toxicology tests before they release the autopsy report.

Yesterday (21.12.09), Los Angeles chief coroner Ed Winter – who investigated into the death of pop star Michael Jackson – insisted the autopsy concluded her passing was not caused by drugs.

The toxicology and neurological results are expected to take up to six weeks to come back.

In notes written by the Investigator Blacklock from the Los Angeles County Coroner’s office, Brittany’s bedroom was filled with “large amounts” of tablets and empty pill packets.

The document stated: “A check of the nightstands revealed large amounts of prescription medication in the decedent’s name. Also noted were numerous empty prescription medication bottles in the decedent’s husband’s name, the decedent’s mother’s name and unidentified third party names.”

Alicia starred in 1995 movie ‘Clueless’ with Brittany – who died of a cardiac arrest at her Los Angeles home aged just 32 – and says while they “didn’t stay in close touch over the years” she’ll always feel love for her.

She said: “I always felt connected to her as we shared a very special experience in our lives together. I feel love in my heart for her – and hope she is at peace. This is truly sad.

“I loved working with Brittany. She was so talented, so warm and so sweet.”

Another one of Brittany’s co-stars, Mena Suvari, paid tribute to her after getting to know her when they worked on 2002 film ‘Spun’.

She tweeted: “I’m just sooo devastated now. Found out Brittany Murphy died earlier this morning + I’m shocked. We worked together + I knew her for yrs.

“And ALL my love to her family and friends. May God be with you in this difficult time. (sic)”

Lindsay Lohan also posted a message after the sad news broke.

She wrote: “My deepest condonlences (sic) go out to all of Brittany Murphy’s loved ones, may she rest in peace… She was a great talent w/a (sic) beautiful soul xo.”

Although she never worked with Brittany, Jessica Simpson spoke of her sadness at the untimely death.

She tweeted: “Brittany Murphy was an incredible ray of light to so many people. Her smile was contagious. My prayers are with her family and loved ones.”

The ‘8 Mile’ actress passed away at the age of 32 yesterday morning (20.12.09) but authorities have already ruled out foul play, despite persistent rumours she was addicted to prescription drugs.

Coroner’s Captain John Kades said: “Foul play is not suspected at this time because there were no outward signs of trauma or other injuries.

“We don’t have an autopsy scheduled yet, but we will do it as soon as possible. Given all the worldwide attention, if we can get it done today (21.12.09), we will.”

However, sources say several prescriptions were found in the house, written for various members of the household including Brittany – who reportedly suffered from diabetes – husband Simon Monjack and the actress’ mother.

One source said: “There were a lot of prescriptions in the house.”

Insiders say the actress had been suffering from flu-like symptoms for days – which she was taking medication for – and had begun vomiting shortly before her death, telling her family she felt “very unwell”.

According to reports, when her mother found her body in the bathroom of her Hollywood home, there was “a significant amount of vomit” in the room.

One of the ‘Clueless’ star’s neighbours said paramedics were in the house for almost an hour before taking her body to an ambulance on a stretcher.

The onlooker said “One paramedic was still working on her. First, I couldn’t even tell that it was Brittany that was on the stretcher. I looked like an older woman. Brittany was completely white and her face was covered with a mask. You could tell that something horrible had happened.”

Police later attended the house for what they have described as a “routine” death investigation.

In addition to the autopsy, toxicology tests to look for traces of drugs and alcohol in Brittany’s system will also be carried out, but the results will not be known for six weeks.

The actress’ family have released a statement about the “terrible tragedy”.

It read: “The sudden loss of our beloved Brittany is a terrible tragedy. She was our daughter, our wife, our love and a shining star. We ask you to respect our privacy at this time.”

Brittany’s estranged father, Angelo Bertolotti – who hadn’t seen the actress in several years – also paid tribute to her.
